| Murray E. Fox,
M.D.

President and
Chief Executive Officer - Ext. 101 Dr. Murray Fox is President and CEO of the Patient
Physician
Network in Plano and Medical Director of the greater than 600 member Plano Physicians
Group.
Dr. Fox received his Bachelors Degree from the University of
Texas in Austin, and his Doctor of Medicine degree from the University of Texas
Health Science Center in San Antonio. His residency training in OB/GYN was
taken at Bexar County Hospital in San Antonio.
Upon completion of his residency, he entered the U. S. Air
Force Medical Corps as a major, serving on the teaching staff at Wilford Hall
U.S. Air Force Medical Center.
Upon discharge from the Air Force in 1975, Dr. Fox entered
the private practice of OB/GYN in Plano.
Since 1984, in addition to his
medical practice, Dr. Fox has focused on organizing the physician providers at
Medical Center of Plano to respond proactively to managed care.
His primary focus over the last
eight years has centered on working with physicians, payors and employers to
understand the challenges of health care delivery. In working with physicians,
Dr. Fox has brought forward the concept of evaluating encounter data to
document the “value” of healthcare to patients, payors and employers.
This process, known as clinical integration, offered the
opportunity to introduce the concept of healthcare reengineering through
assisting physicians to combine quality of care issues with cost, utilization
and outcome data to ensure patient
accessibility, efficiency, education, promotion of wellness and prevention of
disease.
Each physician who is affiliated with the organization has
recognized that their future is linked to the successful establishment and
ongoing operation of a clinically integrated physician delivery system.
Each year, comprehensive
studies are completed and analyzed on various disease entities, then presented
to the various medical and surgical specialties for review and discussion. From
these conferences, clinical efficiencies are realized that positively impact
the overall healthcare of the community.
In addition to his private
practice and work with the Patient Physician Network and the Plano Physicians
Group, Dr. Fox has been a key medical staff leader with the Medical Center of
Plano, serving in the following areas:

Alliance for Healthcare Excellence for significant contributions to the
healthcare delivery system in Collin County.

| Ralph
E. Cross, FACHE |
Senior Vice President and Chief Operating Officer -
Ext. 102
Appointed Senior Vice President and Chief Operating Officer
of the Patient-Physician Network in 1999, Ralph E. Cross oversees the
day-to-day operations of the corporation. During his tenure the
organization has grown from 288 to over 600 physicians in all medical
and surgical specialties, and is generally recognized as the preeminent
physician organization in Collin County. He has served as Chief Operating
Officer of the Methodist Hospitals of Dallas, President and CEO of Fort
Hamilton Hughes Memorial Hospital and Healthcare Corporation in Hamilton,
Ohio, Senior Vice President of Integrated Healthcare Resources International,
and as the Healthcare Consulting Practice Leader with Sweetwater Health
Enterprises. After receiving his Bachelor of Science degree from Florida State
University, Mr. Cross earned a M.B.A. degree with specialization in healthcare
administration from George Washington University, and a M.S. degree in International
Management from the University of Texas. Mr. Cross has served as a speaker and
faculty member at programs and symposiums sponsored by the American Medical
Association, American Hospital Association, the American College of Emergency
Physicians, and Texas Medical Association. He served three terms as Vice Chairman
of the American Medical Association's Commission on Emergency Medical Services,
and was the first non-physician ever elected to an office of the AMA. He is a fellow
in the American College of Healthcare Executives. He is editor of the text "Working
Effectively with Managed Care Plans-Strategies for Success", co-editor of the text
"The Hospital's Role in Emergency Medical Services Systems", and author of twenty
articles in professional healthcare journals.

Scott M. McKnight

Vice President and Director of Information Systems -
Ext. 105
Scott joined the PPN in October
1999 as Vice President and Director of Information Systems. His duties include
the design and implementation of a whole life cycle
information system for the purpose of clinical integration to
help the hospital and physicians make rational choices based
on better insight into the consequences of those choices.
Furthermore, he is responsible for all aspects of technology
from system analyses which includes purchasing, installing,
configuring and maintaining all software and hardware
equipment to database administration which includes designing
and maintaining a normalized relational database system for
both the SQL and Website servers to scripting in Structured
Query Language queries and stored procedures for analyses and
reporting at the PPNHCo. Prior to his current position, Scott
served for six years as the Manager of Information Development
for the former Harris Methodist Health Plan in Arlington,
Texas, where he used statistical modeling of financial and
quality indicators to produce reports and analyses on all
aspects of HMHP/Select performances. Scott received a Bachelor of Arts degree
from
Texas State University in San Marcos, Texas in 1989.

| Cheryl
J. Smith
| Vice President and Director of Provider Relations -
Ext. 104
Cheryl joined PPN in May 1997 and serves as Vice
President and Director of Provider Relations. Her duties
include working closely with the physicians and their staffs
ensuring effective communication to adequately monitor operational
efficiency, payor issues, connectivity, and data reporting. In
addition, she recruits new physicians to PPN by meeting with them
and introducing PPN's many qualities and member benefits. Prior to
PPN, Cheryl served as the Contract Administrator for Foundation
Health Plan where she supervised Contract Coordinators in Texas.
One of her main objectives was to develop a database to track all
physician, hospital, and ancillary contracts on a statewide basis,
which was later adopted corporate-wide. Cheryl was
elected to serve on the Alliance for Healthcare Excellence Board
beginning in January 2007 for a three year term.

Tricia M. Thompson

Vice President of Administrative Services and Director of Credentialing -
Ext. 107
Tricia joined the PPN staff in June 2006 as the Vice President
of Administrative Services. Currently, she is managing the credentialing
program as well as other responsibilities including coordination of Payor
Compliance activities and the Collin County Employer Healthcare Forum.
Before her current position, Tricia was a manager for three years at Texas
Back Institute where she was responsible for the managed care contracting,
credentialing and HIPAA compliance. Tricia also worked at Aetna Healthcare
for ten years in various roles including Regional Provider Communications
Director and Provider Relations Manager. Tricia received her Bachelor of
Science degree from Stephen F. Austin State University in Nacogdoches, Texas.

E. Bazant |
Assistant Vice President and Financial
Coordinator - Ext. 111
Lesley joined the PPN in December 1998 as Assistant Vice President and
Financial
Coordinator. Her duties include all aspects of finance and
accounting for the PPN and PPG. Prior to her current position,
Lesley has over 25 years experience in accounting and banking, working
for various CPA firms and financial institutions.

M. Burris |

Sue has been with PPNHCo since
it’s inception in 1996 as
Assistant Vice President and Coordinator-PPG. Her primary
responsibilities are to address any membership items, functioning as a liaison between the PPG membership, payors, and
the PPN. Prior to joining Plano
Physicians’ Group, Sue was an office manager for seven years for a local
dermatologist who was also a PPG member.
Throughout the years she was involved with PPG’s Office Manager organization
at various levels. She brought with her
a wide variety of knowledge of the functions of a physician’s office. Entering healthcare on the edge of
development for Managed Care, Sue has an understanding of the
difficulties and changes experienced by the PPG membership. Sue is familiar with CPT coding guidelines,
understanding EOB’s and an empathy for the physician and office staff
regarding the
difficulties of the ever-changing healthcare industry.

N. Moore |
Assistant Vice President
and Information Specialist - Ext. 106
Paul joined the PPN in January
2000 as Assistant Vice President
and Information Specialist. His duties include working
with physician practices to ensure compliance with clinical
integration requirements, serving as webmaster of the PPN
website, filling the role of HIPAA Privacy Officer for the
company, and serving on the Payor Compliance Task Force. Prior to his
current position, Paul served for five years as a Pharmacy
Relations Representative for the former Harris Methodist
Health Plan in Arlington, Texas. In this capacity, he prepared
pharmacy data analysis for senior management and was
responsible for the Harris Methodist pharmacy network
expansion for both HMO and PPO product lines in East Texas. Paul
received a
Bachelor of Arts degree from Austin College in Sherman, Texas in
1987.

A. Turney |
Credentialing Coordinator - Ext. 103
Judy joined the PPN staff in April 2007 as Credentialing Coordinator. She has eleven years of
credentialing experience. Prior to joining PPN, Judy worked as the Credentialing
Coordinator for Texas Cancer Associates and has also worked as Senior
Credentialing Analyst with Great West Health Care. |
